MPLP Forms

Statewide Appeals Docket form - We publish a Statewide Appeals Docket quarterly. Please use this form to notify us of any appeal as soon as they are approved.

What should be submitted? Basically, any appeal that LSC would require management approval for, including appeals from District to Circuit Court and from an administrative agency to Circuit Court. If in doubt, just submit it.

New Employee form- We need to know whenever a new staff member joins your office or program to ensure they are added to the Desk Reference and the statewide email address book. We will also send a welcome packet of information to the new staff member that includes: a desk reference, the latest newsletter, and information on MPLP services, taskforces and trainings.
