Issue Summary:

Day care aides and relative care providers are now required to provide verification of identity along with verification of age and a valid Social Security number. Providers wishing to change types will need to fill out new forms. Relative care providers must report any changes in adults living in the household.

What's Happening?

Day care aides and relative providers are now required to provide proof of identity along with verification of age and Social Security number. A new FIA-220 must be completed by an already enrolled day care aide or relative provider if he/she wants to be enrolled as a different type of provider. Relative care providers must report any change in the adults living in their homes so that a check can be run on the Central Registry.
