The Family Independence Agency (FIA) has issued policy clarifying that it can accept the supplemental SER application (FIA-1514) via fax for applicants who already have an active FIA case.

What's Happening?

FIA has issued policy clarifying that applicants who already have an active FIA-1171 on file can fax their supplemental SER application to FIA.

What Should Advocates Do?

Advocates should have supplemental SER applications at their organizations to distribute to clients. Advocates may fax those supplemental applications to FIA for their clients to assure that the application gets to FIA as soon as possible. The fax numbers for all local FIA offices are available online at http://www.mfia.state.mi.us/compositedir/. Advocates should remember that FIA will not accept faxed applications for SER for individuals who do not already have an active FIA case. FIA will not accept faxed FIA-1171 applications (purple booklet) for any program except Food Assistance. Advocates should advise clients to always keep a copy of any applications or documents that they fax, mail, or turn in to FIA.

What Should Clients Do?

Clients who have access to a fax machine and who already have an active case with FIA may want to fax the supplemental SER application to FIA so the application is processed as soon as possible. Clients should always keep a copy of any applications or other documents they fax, mail or turn in to FIA.
